School Closure Release Form
Family Futures Resource Network has the best interest in mind for the children in our care. Supervision of the children registered in our school closure program is the responsibility of our school closure program staff members. We are only responsible for your children during camp program times.
At Family Futures Resource Network we strive to make every experience positive for the children in our camps. We reserve the right to remove any child from a camp at any time.
1. School Closure Program Hours:
We are not responsible for your child(ren) before and after program hours. Family Futures Resource Network is not responsible for any children who arrive at non-designated camp program times. It is the responsibility of the parent to make sure their child(ren) is picked up at the end of the camp program time. Staff cannot walk or drive children home, nor arrange alternative transport.
School Closure Program Time:
School Closure for Ages 6 - 12
Drop-off 9:00 AM – 5:00 PM
Parents/caregivers must sign their child(ren) in and out at drop-off and pick-up times. If someone other than you will be dropping off or picking up your child(ren), please ensure their information has been provided as part of your profile. Please also let them know to to sign your child(ren) in/out of the program and that they may be asked to provide photo I.D. to prove identity. If you are permitting your child to leave the program alone, please see Permission for Child to Leave Program Unaccompanied below.
2. Before/After Care and Late Policy
Before care is available from 8:30 AM - 9:00 AM and after care is available from 5:00 PM - 6:00 PM If you require before/after care,
If you selected NO above, you must not drop off your child(ren) before 9:00 AM, and must arrive to pick your child(ren) by no later than 5:00 PM
If you have selected YES above, you cannot drop off your child before 8:30 AM. and must arrive to pick up your child by no later than 6:00 PM If you arrive later than 6:00 PM you will be charged a $25.00 flat fee for every time you are late. If you are late repeatedly, your child(ren) may not be permitted to return to camp.
3. Permission for Child(ren) to Leave Program Unaccompanied
Family Futures Resource Network is NOT responsible for children after the program hours and recommends that parents/caregivers sign their children in and out. However, if you permit your child(ren) to leave unaccompanied after the program has finished at 5:00 PM,
Your selection above confirms you understand that Family Futures Resource Network is not responsible for your child(ren) once they leave the program location.
4. Program Photo/Video Release Form
Under the Freedom of Information and Protection of Privacy Act, it is necessary for Family Futures Resource Network to obtain consent in order to use photos and/or video footage of people for the purposes identified below.
Collection and Use
The photos and/or videos taken during this time period will be used to promote Family Futures Resource Network and the programs and services we offer. Your consent gives Family Futures Resource Network the right to use these images in different media. These may include but are not limited to newspaper ads, brochures, newsletters and other print materials and on the Agency’s web site. Though the images will be used only by Family Futures Resource Network, the materials may be distributed elsewhere.
Consent – Personal
I hereby waive any rights to inspect or approve the use of photographs or video footage taken of me or my children by photographers working on behalf of Family Futures Resource Network or by Agency staff, and hereby consent to the uses specified above in the collection and use section.

5. Off-Site Information
During school closure day camps we may use the areas below within walking distance of the program location (dependent upon weather):
- Community Playgrounds and Spray Parks
- Local businesses
Please ensure that your child has appropriate clothing (sweaters, coats, snow gear if applicable), sun hat, sunscreen, bug spray, water bottle and lunch/snacks for each day of the program.
6. Program Payment and Cancellation Policy
Full payment of applicable program fees is required upon registration – either by credit card at time of online registration, or within one week (7) days of online registration at either of our offices listed below during office hours:
Summerside Site, 4341 Savaryn Drive SW, Edmonton, T6X 2E8.
Westbrook Site, 25 Fairway Drive NW, Edmonton, T6J 2S6.
**ALL PROGRAMS MUST BE PAID FOR IN FULL PRIOR TO THE START OF THE PROGRAM.**
- You must notify Family Futures Resource Network a minimum of seven (7) days prior to the start date of the program if you wish to cancel your registration.
- A $25 administration fee is included in the program fees and is non-refundable.
- Cancellation requests must be made over the telephone by calling 780 413-4521 Monday – Friday 9:00 a.m. - 4:30 p.m. (closed for lunch between 12:00 – 1:00 p.m.).
- No refunds will be issued less than seven (7) days prior to the program start date. Refunds will not be made for participant no shows.
- Family Futures Resource Network reserves the right to combine, postpone or cancel programs due to insufficient registration. Program cancellations may occur due to reasons beyond the control of Family Futures Resource Network. Full refunds of deposits will be issued if the program is cancelled due to insufficient registration.
7. Confirmation of Program Release Form
I understand that I am responsible for the safety of my child(ren) before and after camp hours (including before and after care if requested).
Having entrusted the care of my child(ren) to Family Futures Resource Network staff, I hereby authorize them to obtain any emergency care required.
